This policy is an insurance contract covering the below given conditions and attached English private conditions.

POLICY MATURITY DATE: 1 year starting from the issue date.
SUBJECT OF INSURANCE:
This individual accident policy; provides security limits and within the attached English conditions against physical damages for individuals under the supervisory and control of the insured .

A)
Accident and illness treatment expenses: Maximum USD.50,000 and valid for more than USD.250 exclusion
Exclusion: USD.250
* Within the limit of the accident and illness treatment expenses, this includes the ear and lung treatment expenses with the pressure room expenses for divers
B)
1. In case of decease : USD.15,000
2. Full functionless of both eyes : USD.15,000
3. Functionless of a single eye: USD.15,000
4. Disability of organs (arms and legs) functionless of both:USD.15,000
5. functionless of a single organ (arms and legs): USD.15,000
6. functionless of a single eye and a single organ:USD.15,000
7. ýn case of full stable disability ( loss of a single eye or two eyes that can never be medically cared or loss of arms or legs single or both together):USD.15,000
C)
Transportation and accommodation expenses: Maximum USD.2,500

CONDITIONS:

• It is given depending on the attached original conditions. Misunderstandings or faults caused by translation will not be considered, The English conditions will have the priority.
  
• It covers the native and foreign divers with condition to obey the rules of insurance policy registered or obtained ticket at schools and some other associations.
  
• It covers inland and abroad diving.
  
• This is an obligatory insurance for all diving schools and all divers must get this insurance registered at such schools. For this reason the premium must be added in the ticket prices.
  
• The diving must be made under supervision of the trainer having PADI, Dive master or equivalent certificate
  
• The security never covers the individual diving.
  
• Maximum depth will be 30 meters.
  
• Only the day light diving are included in the security
  
• The security starts with the delivery of the diving equipment to the diver by the diving school and ends with the return of the diving equipment to the diving school.
  
• It works after all present insurances and it never includes the securities of them.
  
• Trainers having PADI or equivalent certificate are included to this certificate without any premium.
  
• The policy will be given to the diving schools annually and no other policy will be issued for the divers.
  
• Some diving schools may start diving from the beach or coast. The said beach and pool diving are included in the security.
  

JUSTICE AND LEGISLATION: Turkey
* This policy includes foreign exchange.
*on the maturity date of the policy, additional premium collection will be made according to the performed diving quantity.

Premium payment condition: The premium of the policy will be paid 100% in cash
In case of unpaid polices, the cancellation of the policy will be subject of matter.
For this policy/contract, the calculated premium amount (advance payment and partial payments) will be estimated via the exchange rates for Turkish liras given at Akbank Inc. on the issue date.

The advance payment and the partial payments are written on the policy and those will be collected via calculation the exchange rates for Turkish liras given at Akbank Inc. on maturity date.

The amounts written in Turkish Liras on the policy on the issue date will be recalculated on the maturity date of the partial payments for any possible increase of exchange rates and the exceeding amount will be collected via a amendment of collection.
