Required documents and processes:

• The insured must apply to our agency regarding the accident within 5 working days
  
• Policy and receipt copy
  
• Accident report
  
• Alcohol report
  
• Drivers license photocopy of the driver at that moment
  
• Vehicle license photocopy
  
• Pictures of the accident
  
• Tax I.D. card for individuals and Tax Plate copy for companies
  
• If the vehicle belongs to a company, copy of signature declaration
  
• If the vehicle is purchased by credit, a letter of confirmation from the relevant authorities that the payment can be made to this person.
  
• Accident declaration of the insured.
  
• The original invoice given after mending, or invoice copy for company vehicles or commercial vehicles
  
• The traffic policy copies of the vehicles that interfered to the accident.
  
• Application letter to the relevant authorities regarding stolen vehicles, if the vehicle cannot be found within 30 days after incident, a letter from Security department or , "registration is deleted and stolen" letter from traffic department
  
• In case of stolen vehicle, all original keys of the vehicle
  
• Police station report for radio, or cassette player burglary
  

NOTE:
1. After burglary of radio, cassette player, the brand name of the new purchased radio , cassette player must be informed to our agency. Otherwise, in the next burglary our agency will not be responsible of the incident of burglary and damages of radio cassette player. This is valid for all post-mounted accessories.
2. The discount percentages performed by the companies may vary for some damage limits.

IMPORTANT: The documents related to damage obtained from legal offices must bear signature and stamp with "Same as original"
