  o Family civil registration chart
  o Report or declarations regarding the accident
  o Report arranged by SSK (Social Insurance Association) inspectors showing the fault rates
  o The last 6 months payment report of the employee
  o Payments made to inheritors of the deceased by S.S.K.
  o If there is any court case is started by inheritors against either employer or S.S.K., the case petition and the reply must be presented
  o The court decree if the cases are resulted, legal action order and receipt regarding payment of the indemnity that is decided.
  o With permission of the insurance company, acquaintance issued by notary for payments to inheritors
  o Tax I.D. card for individuals and Tax Plate copy for companies
